TSSAA Physical Form Complete Pages 2 & 3 BEFORE seeing doctor. The doctor will complete (AND KEEP) PAGE 4. Dr will complete (AND RETURN TO YOU) PAGE 5.  Upload PAGE 5 into Final Forms. 

Physicals must be dated AFTER April 15th of the current year. 

Physicals can be completed at your primary care provider, walk-in clinics, or at KOC, TOC, etc.
