Concussion Form Sign, Initial, and Return PAGE 3

Sudden Cardiac Arrest Form Sign & Date & Return PAGE 2

Participation & Medical Consent Form Complete, Sign, Date & Return. Please add the player's SSN to the top right corner of the form. It is needed for filing claims. These will be shredded each season. Be sure to include the health insurance information - I've taken more than one player for Urgent Care where this form IS required.

TSSAA Physical Form Complete 1st page BEFORE seeing doctor. The doctor will complete PAGE 2. Turn PAGE 2 in to Coach Chris. 

To the best of my knowledge, there will NOT be a free physical day at McNeeley's this year. Please make arrangements with your own health care provider to get a physical. IF your insurance won't pay for another physical until x date, please do the following:

1) Get a copy of the most recent physical from your physician, provide us with that, and schedule a physical.

2) Provide the date on which you've scheduled the upcoming physical. This will keep us in compliance with the school administration until the current year physical is complete.
